We take the accuracy, completeness and compliance of our medical records very seriously. We require Health Information Management to do the same. This a true data management job : from reviewing patient records to providing regular application support and troubleshooting. It requires a system data expert with great communication skills and sound judgment.
Are you detail:oriented? Do you have experience with medical record review, security and management? Do you enjoy processing data and overseeing its use? We need your skills.
The preferred location for this position is Indianapolis, but other locations will be considered as well.
WHAT WE NEED 13;
Patient record gurus
WHAT YOU WILL DO 13;
:Evaluate potential duplicate patients from across the corporation (acute and ambulatory settings) and determine if they are the same patient. If so, perform patient merge to combine all records.
:Review patient records and research documents as appropriate.
:Identify, aid, and assist in chart correction process.
:Coordinate chart correction activities with hospital or physician practice.
:Provide timely accurate communications and follow ups with all parties involved.
:Review corrections to ensure data integrity is returned to patients information.
:Provide feedback and education to patient registration areas (hospital and physician practice).
:Evaluate incoming Summary of Care reports and determine if the patient exists in our Electronic Medical Record (EMR) system or if a new patient needs to be created. If necessary, create new patient in EMR. Once patient is identified/created, apply documentation to record.
:Review rejected Summary of Care messages sent by Franciscan to other facilities. Troubleshoot rejected reason; mail paper Summary of Care if needed.
:Monitor/Manage paper mailing process for Summary of Care documents that could not be transmitted electronically for the corporation (acute and ambulatory settings).
:Provide regular application support and troubleshooting.
:Participate in application testing.
:Determine functional gaps between new applications and existing applications.
:Provide operational application support post:implementation. 13;
